1. Payment is required at the time of enrollment/registration.
   Registration will not be processed until payment is received.

2. Payment may be made in the form of checks, credit cards, money orders, cash.  We accept Visa and MasterCard.

3. Written cancellation requests received up to 2 weeks prior to program date will be refunded at 75% of the registration fee or full credit issued to attend another CHSER program within the same calendar year.

4. NO REFUNDS DURING THE TWO WEEKS PRIOR TO EACH PROGRAM DATE.  However, if requested, full credit will be issued for one year to attend another CHSER program.

5. $30.00 service fee is charged for checks returned due to insufficient funds.

6. Please advise CHSER at the time of registration if you require special assistance and the specific assistance needed.

7. CHSER maintains a database of certifications and CE hours for attendees.  Please advise CHSER of any contact changes from your original registration information.

8. CHSER does not make arrangements for individual lodging.
   Each participant is responsible for his/her accommodations.
